A 32 years old male patient, with chronic back pain. 3 months prior to the MRI scan, the pain intensified, combined with fever and inability to stay in a supine position. The presence of HLAB27 antigen in the serum was also noted. Visualized ankylosing arthropathy on the level of synovial part of the sacroiliac joints (grade 4 New York criteria), anterior sacroiliac ligament – ossified, posterior sacroiliac ligament – hyperintense on T2wi sequence (reactive enthesopathy/enthesitis). Moderate swelling of the adjacent gluteal muscles on both sides (myositis).
