Ascending aortic replacement

A 62-year-old female patient presented with an ascending aortic aneurysm and severe aortic regurgitation. The Bentall procedure was performed: aortic valve and ascending aorta replacement with a valved conduit. The valve leaflets are excised. The aortic tissue in the 3 aortic sinuses is excised. The coronary arteries are separated from the aortic sinus with small buttons of the full-thickness aortic wall. Pledgeted sutures placed through the aortic annular tissue (intraoperative photograph).
