Bulbar variant progression

Here we have a case of Bulbar variant/progression from Chiari type 1 to 1.5 malformation, block vertebra. A 47 years old female patient, suffered for a number of years and complained of: increased neck pain, paresthesias in arms & back pain. Low lying cerebellar tonsils and obex of the medulla oblongata, which extended below the margins of the foramen magnum by 12.5 mm. On the level of C1-D2 segments of the spine, we see increased spinal cord diameter due to severe dilatation of the central canal - syringohydromyelia, filled with CSF (cerebral spinal fluid ) and has heterogeneous signal due to flow artefacts, adjacent parenchyma is compressed. Note also hypoplastic C2-C3 intervertebral disc – block vertebra (vertebral anomaly/anatomic variant). Degenerative changes of the intervertebral discs.
