Penetrating aortic ulcers

Penetrating aortic ulcer is defined as ulceration of an aortic atherosclerotic plaque penetrating through the internal elastic lamina into the media. A 69-year-old male patient presented with ascending and descending thoracic aorta aneurysms. Maximal ascending and descending thoracic aorta diameter was 56 mm and 51 mm, respectively. Contrast-enhanced CT, including multiplanar reformations, revealed localized ulceration, penetrating into the aortic wall in the tubular ascending aorta and distal third of the descending thoracic aorta. Descending thoracic aorta dilatation represented a pseudoaneurysm containing thrombus, secondary to penetrating ulcer.
