Severe bicuspid aortic valve stenosis

A 55 years old patient with сongenital heart disease, bicuspid aortic valve complained of frequent pain behind the sternum, shortness of breath during a little physical activity & vertigo. Severe aortic valve stenosis was revealed during the transthoracic echocardiography. Intraoperative transesophageal echocardiography before the surgery. ME AV SAX (mid-esophageal, aortic valve, short axis) view. It's the left ventricle systole. The aortic valve is bicuspid. The cusps are thickened, they have inclusions of large calcifications, there is an acoustic “shadow” in the margin zone.
