Optical coherence tomography (OCT) scan of a 61-year-old female patient with stage 3 fully developed macular hole. Vitreous is completely detached from the macula, which persists adhered to the papillae. The operculum is lying at the posterior vitreous a short distance anterior to the retinal surface. The edge of the hole has been thickened by cystic spaces and the photoreceptors are elevated. BCVA 20/63
