Ulcerations of the lower leg

An 81 years old male patient suffered an infection of his left leg. A severe bacterial infection of the left lower leg with arterial and venous insufficiency on CDFI (Color Doppler Flow Imaging) was noticed during the examination. The lower leg is hard on touch, with fibrosis of the dermis and hypodermis & large fields of white atrophy (Atrophia Blanche). On large areas of skin, there is desquamated extinct yellowish cell detritus, which covers the hard purple-coloured skin. Above the line of the talocrural joint, there is a large open oval ulceration, the bottom of which is completely covered with whitish pus and granulation-necrotic tissue.
